Thank you for asking. This is a common question, and it’s important to understand what Medicare covers before you need the medication.

Does Original Medicare Cover Ozempic?

No. Original Medicare does not cover Ozempic.

Ozempic is an injection you give yourself at home. That makes it an outpatient prescription drug. Medicare Part A covers hospital stays. Medicare Part B covers doctor visits and medical supplies. Neither Part A nor Part B pays for outpatient drugs like Ozempic.

That’s where Medicare Part D comes in.

What Ozempic Is Covered for Under Part D

Medicare Part D plans cover Ozempic only when your doctor prescribes it for an approved medical reason. According to the Medicare Prescription Drug Benefit Manual (CMS.gov), Part D currently covers GLP-1 drugs like Ozempic prior authorization for:

  • Type 2 diabetes management
  • Cardiovascular disease risk reduction in people with obesity or overweight. This applies to Wegovy, the higher-dose version of semaglutide, which the FDA approved for this use in March 2024
  • Chronic kidney disease combined with Type 2 diabetes. The FDA approved Ozempic for this in January 2025

If your doctor prescribed Ozempic for Type 2 diabetes, your Part D plan should cover it.

How to Get Ozempic Coverage With Medicare

If you want Ozempic, you need prescription drug coverage. You have two ways to get it.

Option 1: Add Medicare Part D

If you have Original Medicare, you can add a stand-alone Medicare Part D prescription drug plan. Most plans include Ozempic on their formulary (list of covered drugs). However, not all plans cover it. You must check before enrolling.

Option 2: Choose a Medicare Advantage Plan With Prescription Drug Coverage

A Medicare Advantage Prescription Drug plan combines hospital, doctor, and prescription coverage into a single plan. Many plans start at $0 per month. These plans usually include prescription drug coverage and may cover Ozempic.

What About Ozempic for Weight Loss?

Here’s where the rules get more specific.

Medicare law has long excluded drugs used only for weight loss. But something changed in 2026. CMS launched the Medicare GLP-1 Bridge,  a temporary program that grants Medicare beneficiaries access to certain GLP-1 medications for weight management at a $50 copay per month through December 31, 2027. Wegovy and Zepbound are covered. Ozempic is not.

The Cost of Ozempic Without Coverage

Without insurance, Ozempic is expensive. According to NovoCare, the list price is around $968 per month, about $12,000 per year. Actual costs depend on your dose.

There is good news on the horizon. In January 2025, CMS selected Ozempic, Wegovy, and Rybelsus for Medicare drug price negotiations under the Inflation Reduction Act. Negotiated prices take effect on January 1, 2027. This may lower your out-of-pocket costs for semaglutide through Medicare Part D.

When Will a Generic Be Available?

Not soon. There is no generic Ozempic today. According to the U.S. Food and Drug Administration (FDA), Novo Nordisk’s patent on Ozempic does not expire until December 2031. Until then, you pay the brand-name price.

Bottom Line

Medicare Part D covers Ozempic in 2026 when it’s prescribed for approved medical conditions such as Type 2 diabetes, but not when it’s used solely for weight loss. Ask your doctor about possible side effects before starting Ozempic. 

Medicare’s temporary GLP-1 Bridge assistance program covers Wegovy and Zepbound for eligible Medicare beneficiaries who use these medications for weight management.To qualify, your healthcare provider must submit clinical documentation showing your BMI meets the program’s requirements. You may also need to meet additional health condition thresholds, which sometimes allow lower BMI levels. Your provider will also need your agreement to make lifestyle changes through diet and exercise while taking the medication.
